Clinical Care Coordination
Team CareMed Specialized Services Approach
The CareMed Pharmacy Team works diligently to help improve the quality and cost of care by coordinating specialty pharmacy activities. By providing ongoing support for patients with chronic conditions, together, they can achieve the best possible outcomes from treatments.

CareMed ensures the patient’s prescribed therapy needs are met promptly, accurately, consistently and compassionately. The professional healthcare team is comprised of experienced and qualified clinical pharmacists, nurses and pharmacy technicians dedicated to fulfilling prescriptions for patients undergoing complex therapies.

Clinical Care Coordination
Each patient is assigned a Clinical Care Coordinator (CCC). The CCC's will contact the patient each week to follow-up on refills and assure timely medication deliveries so that treatment continues uninterrupted.

CareMed’s clinical pharmacists and nurses are available to answer any and all questions 24 hours a day, 7 days per week, and 365 days per year. Collaboration between healthcare providers, patients and managed care organizations ensures the highest level of quality care is attained, resulting in increased patient compliance and effectiveness of therapies.

Outlined here are the roles that each professional plays:

Clinical Care Coordinator (CCC)
  • Enrolls patient and provides welcome materials
  • Schedules activities for follow-up
  • Places proactive refill calls to patient 10 days prior to need date
  • Contacts physician or referral source for missing information if necessary
  • Assists patient in arrangement of homecare if needed
  • Reviews patient checklist
  • Performs follow-up assessment when due, according to disease protocol
  • Contacts physician, if necessary
  • Answers and documents patient’s questions
  • Assesses medication inventory to minimize waste and control costs

Clinical Pharmacist (RPh/Pharm-D)
  • Evaluates initial referral and ensures clinical guidelines are met
  • Obtains patient weight and verifies appropriate dosing
  • Reviews continued medical necessity with each refill
  • Assists physician with dose adjustment
  • Assists patient with all medication questions
